Terms and Conditions for HEART Helping Every Animal Rehabilitation Therapy Effective Date: 1st January 2025 These Terms and Conditions ("Agreement") govern the provision of physiotherapy services ("Services") by HEART ("Therapist," "We," or "Us") for your animal ("Animal"). By booking or receiving Services, you ("Client," "You") agree to these Terms and Conditions.

1. Booking and Appointment Confirmation

1.1 All appointments must be scheduled in advance, and confirmation is required.

1.2 You are responsible for providing accurate details about your Animal’s condition and health status during the booking process.

2. Cancellation Policy

2.1 Cancellation or Rescheduling: If you need to cancel or reschedule an appointment, you must notify us at least 48 hours in advance.

2.2 Late Cancellations/No-Shows: Cancellations made less than 48 hours before the scheduled appointment or failure to attend without notice will result in a 50% cancellation fee based on the full appointment cost unless the appointment can be filled.

2.3 Rescheduled Appointments: Rescheduling within 48 hours will be subject to the same cancellation policy, and additional charges may apply.

3. Payment Terms

3.1 Payment for Services is due at the time of the appointment, unless otherwise agreed.

3.2 Payments can be made via cash or BACS transfer.

3.3 Any outstanding invoices not settled within 7 days of the appointment will incur a late fee of 2.5% per day until paid in full.

3.4 We reserve the right to suspend or refuse further Services if payment is not received within 7 days of the original appointment date.

4. Animal Health Disclosure

4.1 You must fully disclose all relevant health information about your Animal at the time of booking and prior to each appointment. This includes, but is not limited to, medical conditions, injuries, surgeries, medications, allergies, or any other factors that may affect the Animal’s ability to safely undergo physiotherapy treatments.

4.2 The Therapist is not responsible for any complications, injuries, or adverse outcomes that result from a failure to disclose important health information regarding your Animal.

4.3 The Therapist may refuse to perform Services if, in their professional opinion, the Animal’s health condition may pose a risk to its wellbeing during treatment.

5. Liability

5.1 The Therapist will provide Services with reasonable care, skill, and diligence. However, the Therapist cannot guarantee specific outcomes or results, as each Animal’s response to treatment can vary.

5.2 You agree to indemnify and hold the Therapist harmless from any liability, claims, or damages arising from any undisclosed health conditions or failure to follow recommendations provided during treatment.

5.3 The Therapist is not liable for any injuries or health complications that arise as a result of neglect, improper care, or failure to follow prescribed treatment protocols.

6. Consent to Treatment

6.1 By accepting these Terms and Conditions, you confirm that you consent to the physiotherapy treatments provided to your Animal.

6.2 If at any time during the course of treatment, you wish to stop or alter the treatment plan, you must inform the Therapist immediately.

7. Confidentiality

7.1 The Therapist respects your privacy and will keep all information shared during the course of the treatment confidential.

7.2 Any records created during the treatment process are maintained in accordance with applicable laws regarding privacy and data protection (GDRP 2018)

8. Changes to Terms

8.1 The Therapist reserves the right to update or modify these Terms and Conditions at any time. Any changes will be communicated to you, and your continued use of the Services constitutes acceptance of the revised terms.

9. Governing Law

9.1 These Terms and Conditions are governed by and construed in accordance with the laws of England. Any disputes will be resolved under the jurisdiction of the courts in England.
